深板层角膜移植术治疗圆锥角膜的临床效果

The clinical application and curative effect of deep lamellar keratoplasty for keratoconus

目的评价深板层角膜移植术治疗圆锥角膜的临床疗效。方法采用深板层角膜移植术对我院收治的21例（23眼）圆锥角膜进行治疗，对其疗效进行观察分析。术后随访6～24个月。结果术后矫正视力〈0．1者1例（1眼），0．3者2例（2眼），0．4～0．7者15例（16眼），〉0．7者3例（4眼）；发生排斥反应3眼，植片透明22眼，植片半透明1眼。后弹力层皱折5眼，后弹力膜穿孔2眼。结论深板层角膜移植术为治疗圆锥角膜提供了一个更安全有效的选择，具有术。后视力较好、并发症较少、对供体材料条件要求较低等优点。

Objective To review the efficacy of deep lamellar keratoplasty in treating keratoconus, Methods We reviewed the 21 patients （23 eyes） who had treated by the deep lamellar keratoplasty during 2003 to 2006 in our hospital. The effectiveness of operation was clinical observed and evaluated, The follow up time was 6-24 months. Methods 16 case （17 eyes） due to degenerative myopia macular hole retinal detachment was treated with Vitreoretinal surgery, filled with C3F8. We investigated the visual acuity, vis- ual field, OCT, ocular fundus , complication and followed up from 6 to 18 months （average 12, 4 months）. Results Postoperative best corrected visual acuity lower than 0.1 in 1 eye ,0.3 in 2 eyes of 2 cases,0.4 N 0.7 in 16 eyes of 15 cases and 0.7 or better in 4 eyes of 3 cases. The graft kept clear in 22 eyes of 21 cases and translucent in 1 eyes of 1 case. Reject reaction occurred in 3 eyes of 3 cases. Condusions Deep lamellar Keratoplasty provides a safer and successful alternative to penetrating keratoplasty for kera- toconus. Treatment of keratoconus with deep lamellar keratoplasty has better corrected visual acuity , lower complication rates and lower requirement for donor,
