改良泪囊鼻腔吻合术的临床观察

Clinical observation of improved dacryocystorhinostomy for treatment of chronic dacryocystitis

目的观察改良泪囊鼻腔吻合术治疗慢性泪囊炎的效果。方法256例(281眼)施行改良式泪囊鼻腔吻合术,即术中不切断内眦韧带、骨窗较大且低位、吻合腔填塞明胶海绵、只吻合泪囊前叶并悬吊固定于泪前嵴骨膜上、必要时于泪道内置入硅胶软管。结果术后随访6月至3年,治愈273眼(97.15%),有效6眼(2.14%),无效2眼(0.71%)。结论改良泪囊鼻腔吻合术具有操作简便,成功率高,是治疗慢性泪囊炎有效的方法。

Objective To observate effect of treatment for chronic dacryocystitis by improved dacryocystorhinostomy. Methods 256 cases（281 eyes） were operated with improved dacryocystorhi- nosrtomy, that is with inner canthal ligament not been cut off; windowing wide and low. only anastomosing anterior lobe of lacrimal saccus, suspending it on bone membrane of anterior lacrimal crest, anastomotic cavity stussed with gelatin sponge, implanting silicone tube in ifnecessary. Results To ohaervate it from 6 months to 3years after operation, that os cured 273 eyes（97.15% ）,6 eyes were improved,2 cases failed. Conclusions With some advantgges such as operated convenienty, successful rate high and patients feeling better,improved dacryocystorhinostomy is effective and depend-able method for treating chronic dacryocystitis.
