清热养阴除湿丸治疗活动期强直性脊柱炎对炎性指标的影响及临床疗效观察

Clinical effect of Qingre Yangyin Chushi Pill on inflammatory index in active stage of ankylosing spondylitis

目的：观察清热养阴除湿丸（半枝莲、虎杖、银花、连翘、土茯苓、白鲜皮、生熟地、白芍、川乌、桂枝）治疗活动期强直性脊柱炎（AS）对血沉（ESR）、C反应蛋白（CRP）的影响及各项临床指标的变化，并以柳氮磺吡啶（SSZ）作为对照，以评价清热养阴除湿丸对AS的有效性和安全性。方法：采用随机分组、对照，将80例活动期AS患者分为两组，观察组56例给予清热养阴除湿丸12g，口服每日3次，对照组24例给予SSZlg，口服每日2次。2月1疗程，共治疗3个疗程，每个疗程结束为一评价点，观测患者ESR、CRP、BathAS活动指数（BASDAI）、BathAS功能指数（BASFI）、晨僵时间等症状体征改善，同时观察药物的不良反应。结果：两组治疗前后ESR、CRP明显下降，症状体征、BASDAI、BASFI、各项观察指标有明显改善，治疗前后差异有统计学意义（JP〈0．05～0．01），两组间比较差异无统计意义（P〉0．05）。总有效率观察组和对照组分别为89．28％和83．33％，两组比较差异无统计意义（JP〉0．05）。结论：清热养阴除湿丸对活动期强直性脊柱炎患者的ESR、CRP下降有明显作用，并能缓解临床症状，不良反应观察组明显低于对照组，且对肝、肾、血液系统无不良影响。

To observe the effection and side effect of Qingre Yangyin Chushipill （QRYYCSP） in treating patients in active stage of ankylosing spondylitis （AS） on ESR, CRP, and physicalsign index. METHODS ： Eighty patients with AS were randomly divided into tow groups, the 56 patients in the treated group were treated with QRYYCSP, and the 24 patients in the control group were treated with sulfasalazine （SSZ）. The period of treatment was two-month, there were three periods of treatment overall. To observe the changes of ESR, CRP, Bath Ankylosing Spondylitis Disease Activity Index（ BASDAI）, Bath Ankylosing Spondylitis Functional Index （BASFI）, morn- ing,stiff, symptoms, signs, and side effect. RESULTS： ESR, CRP, Symptoms, signs, BASDAI and BASFI decreased significantly in both groups after treatment （p 〈0.05 -0.01 ）. There was no significant difference between these two groups. （p 〉 0.05 ）. CONCLUSION： It is indicated that QRYYCSP has better effects on ESR and CRP, it can improve symptoms and signs of patients with AS in active stage, and also can reduce untoward reaction in comparison with SSZ, and has no obvious side effect on liver, kidney and haematology.
