高压氧联合甲钴胺治疗糖尿病周围神经病变的疗效 被引量:6

Effects of therapy of hyperbaric oxygen combined with mecobalamin on diabetic peripheral neuropathy

摘要 目的评价高压氧联合甲钴胺对糖尿病周围神经病变的疗效,探讨治疗糖尿病周围神经病变的有效方法。方法378例糖尿病周围神经病变患者分为2组。治疗组198例,予高压氧联合甲钴胺治疗,高压氧疗法采用大型空气加压舱,治疗压力为0.2MPa(2.0ATA),每日1次;甲钴胺500μg,肌肉注射,每日1次。对照组180例,单独使用甲钴胺500μg,肌肉注射,每日1次。2组患者疗程均为4周。结果治疗组显效率、总有效率分别为40.9%和100%,明显高于对照组的17.3%及53.3%,2组间差异有统计学意义(P〈0.01);治疗组神经传导速度及血流动力学改善均明显高于对照组(P〈0.01)。结论高压氧联合甲钴胺对糖尿病周围神经病变的治疗效果显著,总有效率为100%,可作为糖尿病周围神经病变的常规治疗方法。 Objective To study effects of hyperbaric oxygen combined with mecobalamine on diabetic peripheral neuropathy (DPN) and explore effective therapeutic way to DPN. Methods Three hundred and seventy-eight patients were divided into two groups. The treatment group (198 cases ) were treated with hyperbaric oxygen(HBO) and mecobalamin (0.2 MPa, 500 μg mecobalamin i. m. ,once a day). The control group( 180 cases) were only treated with 500μg mecobalamin i. m. , once a day. Courses of treatment in two groups both were four weeks. Results The effective rate and overall efficacy rate of treatment group were 40.9% and 100%, respectively. They were significantly higher than those of control group ( 17. 3% and 53.3% ) (P 〈 0.01 ). At the same time, there were obvious differences in nerve conduction velocity and dynamics of blood flow between two groups ( P 〈 0.01 ). Conclusions HBO combined with mecobalamin has therapeutic effect on DPN, which may be regared as a conventional therapy.
