摘要
目的:探讨MR双回波成像在成人脊柱病变中的应用。方法:回顾分析本院126例脊柱病变患者,包括31例恶性椎体肿瘤、38例急性椎体良性压缩性骨折、8例血管瘤、26例终板退变、18例局灶性骨髓脂肪变性及5例脊柱感染,计算各种病变正反回波的信号强度比(反相位/正相位,SIR),并对各组病例作对比和分析。结果:各种病变的信号强度比分别为:恶性椎体肿瘤:1.01+0.086;急性良性骨折:0.81+0.085;感染:0.86+0.096;血管瘤:0.54+0.117;终板退变:0.78+0.160;骨髓脂肪变性:0.84+0.210。其中恶性病变与感染以外的良性病变SIR差异均有统计学意义(P<0.05),血管瘤与其他病变SIR差异均有统计学意义(P<0.01),其余各组间SIR差异无统计学意义。结论:双回波成像对脊柱病变的诊断和鉴别诊断有很好的辅助作用。
Objective:To evaluate the use of dual echo MR imaging in adult vertebral lesions. Methods: We retrospectively reviewed 126 cases of adult vertebral lesions, including 31 cases of malignancy, 38 of benign acute compression fracture, 8 of hemangioma, 26 of endplate degeneration, 18 of focal fatty degeneration and 5 of infection, The signal intensity ratios ( SIR, signal intensity at opposed - phase over signal intensity at in - phase) were calculated and compared among these conditions, Results: The SIRs of vertebral lesions were 1. 01 ± 0. 086 for vertebral malignancy, 0.81± 0. 085 for benign acute fracture, 0.86 ±0. 096 for infection, 0.54 ± 0. 117 for hemangioma, 0.78±0. 160 for endplate degeneration, and 0.84 ± 0.210 for focal fatty degeneration. There was a significant difference in SIR between vertebral malignancy and other benign lesions except infections ( P 〈 0.05 ) , as was between hemangioma and other lesions (P 〈0.01 ), but not among the rest of benign lessions. Conclusion: Dual echo imaging was helpful to make diagnosis and differentiated diagnosis of vertebral lesions.
