影响慢性重型肝炎预后相关因素的分析

Factors Related with Prognosis in Chronic Severe Hepatitis

目的:探讨影响慢性重型肝炎预后的有关因素。方法:分析156例慢性重型肝炎患者的预后与年龄、性别、凝血酶原时间、凝血酶原活动度、血清总胆红素、白蛋白、谷丙转氨酶、谷草转氨酶、总胆固醇、胆碱酯酶、甲胎蛋白、并发症、有无肝硬化的关系。结果:年龄、凝血酶原时间、凝血酶原活动度、血清总胆红素、白蛋白、总胆固醇、胆碱酯酶、甲胎蛋白等指标及并发症对慢性重型肝炎的预后有影响(P<0.05),而谷丙转氨酶、谷草转氨酶、性别、有无肝硬化对其无明显影响(P>0.05)。结论:慢性重型肝炎患者年龄、血清凝血酶原时间、凝血酶原活动度、总胆红素、白蛋白、总胆固醇、胆碱酯酶、甲胎蛋白及并发症是判断预后的重要依据。

Objective ：To investigate the factors related with prognosis of chronic severe hepatitis. Methods： In 156 patients with chronic severe hepatitis, data on age, sex, prothrombin time, prothrombin activity, total serum bilirubin, albumin, alanine aminotransferase, aspartate aminotransferase, cholesterol, cholinesterase, alpha-fetoprotein and presence of complications and hepatic cirrhosis were evaluated for their relationship with prognosis. Results： The prognosis of chronic severe hepatitis was shown to correlate with age, prothrombin time, prothrombin activity, total serum bilirubin, albumin, cholesterol, alpha-fetoprotein and presence of complications （P 〈 0. 05 ） but not with alanine aminotransferase, aspartate aminotransferase, sex and hepatic cirrhosis （ P 〉 0.05 ）. Conclusion： Data on age, prothrombin time, prothrombin activity, total serum bilirubin, albumin, cholesterol, cholinesterase, alpha- fetoprotein and complications may be important factors for judgment of prognosis in chronic severe hepatitis.
