摘要
为使绝经后妇女及罹患低雌激素水平的原发闭经及卵巢早衰等病员及早发现骨质疏松,给予适时防治。我院采用Myrial公司生产的Soundscan2000型骨质疏松检测仪,检测患者胫骨长轴超声速度,以评价骨质量,分析预期发生骨折的风险。自1996.1~12,共检测488例,分4组:①20~35岁月经正常健康对照组154例;②约经3a以上280例;③低雌激素性原发闭经28例;④卵巢早衰26例。结果显示:正常人群对照组超声速度平均(4060±386)m/s,明显高于后3组(尸<001),并显示超声速度与年龄及雌激素水平呈高度负相关r=0.634P<0.001。我们认为:这种新型测量密质骨超声速度的方法,具有操作简便,精确度高,便于推广的优点,且无X线幅射,在妇产科领域中防治骨质疏松有一定的临床实用价值。
PURPOSE It's our purpose to evaluate both density and elasticity of bone and assess osteoporotic fracture by using ultrasound instrument Soundscan 2000, the se1ectre measures provide some scientific basis for developing health protection about older women in the field Of obstetrics and gynecology.METHODS A total of 334 patients with postmenopause, amenia and premature ovarian failure and a control groups of 154 volunteers were studied. We use Soundscan 2000 instrument (developed by Myrial Ultrasound Systems Ltd). To measure the longitudinal transmission of ultrasonic waves in cortical bone, with raw data analyzed to produce a single representative speed of sound (SOS) value, the on - screen disp1ay includes a comparison of the patients Z - score, all data is stored and can be displayed on the screen in either tabular on graphical formats.The serum concentrations of FSH, LH, E2, BGP, BAP, CICP the urinary concentrations Ca/Cr, Pyd/Crwere measured in 334 patients and 154 controls.RESULTS The speed of sound (SOS) value in patients significantly decreased in compared with control group, while the patients serum concentrations of FSH, LH, were increased, the E2 significantly decreased, the urinary concentrations of Ca, Pyd were increased in comparison with contro1 group.The drie speed of sound value of total 488 measures is compatib1e with biochemistry measures value.CONCLUSIONS Soundscan 2000 is the first and only ultrasound instrument that is able to assess the fragility of the cortical layer of bone. Because there is negative correlation between the serum concentration of E2 and the bone sound speed value, it's able to make early diagnosis of osteoporosis and beneficial to the prevention and treatment of osteoporosis in the postmenopausal women.
