涂阴肺结核病首诊监督负责制管理的可行性研究

Feasibility study on implementing first visit responsibility for smear-negative tuberculosis patients in county tuberculosis dispensary doctors

目的研究县结核病防治医生实施首诊监督负责制管理模式对涂阴肺结核病患者治疗行为依从性的影响。方法县结核病防治医生对首诊涂阴肺结核病患者实施全程治疗监督管理，并且利用画册进行治疗管理、知识传授与指导。结果实验组100％的研究对象依从县结核病防治医生管理模式，进行自我治疗管理。对照组仅有3．31％的患者依从家庭成员督导服药治疗管理，实验组96．69％的患者依从县结核病防治医生指导的治疗管理方法自我服药。结论县结核病防治医生对涂阴肺结核病患者实施首诊监督管理负责制是适合患者心理需要的一种可行治疗管理模式。

Objective To study the influence of implementing the management mode of first visit responsibility in county tuberculosis （TB） dispensary doctors on treatment compliance of the smear-nega- tive TB patients. Methods County TB dispensary doctors were asked to conduct supervision and management in the whole course of treatment for first visit smear-negative TB patients, and impart and instruct the knowledge of treatment management by using picture albums. Results All the patients in study group were treated by self-management treatment complying with the orders of county TB dispensary doctors. Only 3.31% of patients in control group complied with the treatment mode of supervised medication by family members, and 96.69% of patients in study group were treated by self-medication complying with the instructions of the county TB dispensary doctors. Conclusion To implement first visit responsibility for the smear-negative TB patients in the county TB dispensary doctors is a feasible treatment management mode which meets the psychological needs of tuberculosis patients.
