老年急性白血病临床分析

Clinical Analysis of Acute Leukemia in The Elderly

目的 分析老年急性白血病发病及临床特点，寻求治疗老年急性白血病的有效措施。方法 对初治的老年急性白血病39例进行临床分析，其中60～69岁30例，70岁以上9例，男性30例，女性9例，急性髓细胞白血病（AML）组31例，采用HA、DA、MA、MEA方案对其中24例化疗；急性淋巴细胞白血病（ALL）8例，采用VDCLP方案对其中5例化疗。结果 CR6例，其中AML4例，ALL2例；PR9例，其中AML8例，ALL1例，29例中总有效率为51．72％。结论 老年急性白血病临床上有其特殊性，治疗上更强调个体化，并加强支持治疗，才可望提高老年急性白血病的疗效。

Objective To analyze the clinical characteristics of acute leukemia in elderly patients and look for the effective measures to cure it. Methods Total 39 cases of newly diagnosed acute leukemia of patients aged between 60 - 80 were observed in the study, including 30 between 60 - 69 years old and 9 over 70, male 30, female 9,31 patients were diagnosed acute myelocytic leukemia （AML） and the other 8 were acute lymphocytic leukemia（ALL）. Chemotherapy of HA, DA, MA and MEA regimens were given to 24 cases of AML and VDCLP regimen was given to 5 cases of ALL. Resuits Six patients achieved CR, including 4 cases of AML and 2 cases of ALL; Nine patients achieved PR, including 8 cases of AML and 1 case of ALL. The effective rate was 51.72% of 29 cases. Conclusion Acute leukemia of the elderly people has its specificity in clinical appearances, and the treatment strategy should be made more individually and the supportive therapy should be enhanced. Therefore it is possible to improve the effect of acute leukemia in the elderly.
