DHS内固定加中药内服治疗股骨粗隆间骨折

Femoral intertrochanteric fracture treated with the combination of DHS internal fixation and traditional chinese medicine

目的研究观察动力髋螺钉(dynamic hip screw,DHS)内固定配合中药内服治疗股骨粗隆间骨折的临床疗效。方法自2000年7月至2006年6月应用DHS内固定治疗股骨粗隆间骨折156例,其中单纯DHS内固定86例(对照组),DHS内固定+中药内服70例(治疗组)。中药内服按骨折三期辨证施治,早期以行气消瘀为主,中期以接骨续筋为主,后期以补肝肾为主。结果156例中148例获随访,随访时间8个月～3年,平均20.7个月,其中对照组78例获随访,治疗组70例获随访,骨折愈合时间治疗组为(12.3±2.2)周,对照组为(15.8±2.5)周,两组比较有显著统计学意义(P<0.05);治疗组髋关节功能优良率达90%,对照组为76%,两组比较有显著统计学意义(P<0.05)。结论DHS内固定配合中药内服可有效缩短骨折愈合时间,改善髋关节功能。

Objective To observe the therapeutic effect of the combined DHS（dynamic hip screw） internal fixation and traditional chinese medicine for femoral intertroehanteric fracture. Methods A total of 156 cases of femoral intertroehanteric fracture with DHS internal fixation were collected from Jul 2000 to Jun. 2006, They were divided treatment group and control group. 70 cases in treatment group were treated with DHS and oral medication of traditional chinese medicine, while 86 cases in the control group only with DHS, In treatment group, the diagnosis and treatment on the basis of three period of the course of fracture coalescence. The oral medication can accelerate qi circulated and extravasated blood dissipated in the fast period, and set a bone and muscles in the second period, and benefit liver and kidney in the third period. Results 148 cases in 156 cases were followed up for an average period of 20.7 months, which arranged from 8 months to 3 years. 8 eases in the control group were not followed up. The fracture healing time in the treated group was 12.3±2,2 weeks and that in the control group was 15.8±9.5 weeks, showing significant difference between them （P〈0.05）, The satisfactory rates of hip joint function in treatment group and control group were 90% and 76%, The difference was statistically significant （P〈0.05）, Conclusion It can shorten the fracture healing time and improve the function of hip joint with combined DHS and tradition Chinese medicine,
