强直性脊柱炎患者血小板升高的临床意义

The clinical significance of platelet elevation in patients with Ankylosing Spondylitis(A)

目的探讨强直性脊柱炎患者血小板升高的临床意义。方法回顾性分析和比较187例AS患者中126例活动期AS患者组与61例临床缓解期AS患者组的患者脊柱关节休息痛分数、晨僵时间、血沉、C反应蛋白、PLT等指标;126例活动期AS患者中PLT升高组与PLT正常组的患者脊柱关节休息痛分数、晨僵时间、血沉、C反应蛋白等指标进行比较;17例活动期PLT升高AS患者经过用柳氮磺吡啶(SSZ)、丹参注射液(丹参片)、非甾类抗炎药等治疗,治疗前后患者脊柱关节休息痛分数、晨僵时间、血沉、C反应蛋白、PLT等指标进行比较,对资料进行统计分析。结果患者脊柱关节休息痛分数、晨僵时间、血沉、C反应蛋白、PLT在活动期AS患者组均明显增高(P<0.05);PLT升高AS患者组的晨僵时间明显大于PLT正常AS患者组;PLT升高AS患者组经过用柳氮磺吡啶(SSZ)、丹参注射液(丹参片)、非甾类抗炎药等治疗后,患者脊柱关节休息痛分数、晨僵时间、血沉、C反应蛋白、PLT均显著降低。结论AS患者体内血小板计数的变化与AS疾病活动性有关,可作为AS活动性判断的参考指标之一。

Objective To investigate the clinical significance of platelet elevation in patients with Ankylosing Spondylitis （AS）. Methods Regressively analyzing and comparing the indexes including rest pain score of joint, duration of morning stiffness, exythrocyte sedimentation rate, C-reaction protein and platelet count in the two groups of active and clinical relievement in 187 Ankylosing Spondylitis patients： one group with active AS, and the other with clinical relievement AS. Comparing the indexes including rest pain score of joint, duration of morning stiffness, erythrocyte sedimentation rate and C-reaction protein in the two groups of norreal platelet and elevated platelet in 126 patients of active Ankylosing Spondylitis. And to compare the indexes including rest pain score of joint, duration of morning stiffness, erythrocyte sedimentation rate, C-reaction protein and platelet before and after the therapy of SSZ, danshen injection（danshen tablet） and NSAIDs in 17 active Ankylosing Spondylitis patients with elevate platelet. The statistic analysis of the data is performed. Results Significant elevation is found in the following aspects including press pain score of joint, duration of morning stiffness, erythrocyte sedimentation rate, C-reaction protein and platelet in patients with active Ankylosing Spondylitis （P〈0.05）. The duration of morning stiffness was longer in group of platelet elevation than in the normal one. In the patients of Ankylosing Spondylitis with elevated platelet, the indexes of rest pain score of joint, duration of morning stiffness, erythrocyte sedimentation rate, C-reaction protein and platelet was significanfly decreased after treatment of SSZ, danshen injection （danshen tablet） and NSAIDs. Conclusion The change of platelet in Ankylosing Spondylitis patients is correlative with the activity of disease and could be regarded as an index of disease activity.
