鼻尖皮肤肿瘤切除后鼻唇沟皮瓣修复缺损35例

The reconstruction of the skin defect with nasolabial flap after the tumor of apex nasi resection: 35 cases

目的介绍鼻尖皮肤肿瘤切除术后创面的即刻修复经验。方法从2004年2月—2007年2月,在遵循鼻亚单位组成的美学基础上,根据创面大小、形状、部位及周围皮肤情况选择鼻唇沟皮瓣进行修复35例。其中男21例,女14例,年龄28—66岁,基底细胞癌15例、鳞状细胞癌9例、黑色素痣7例、表皮囊肿5例。恶性肿瘤分期0—Ⅰ期,病程3个月—25年。肿瘤切除后致鼻尖缺损0.6cm×0.8cm—1.5cm×2.0cm。用鼻唇沟皮瓣直接拉拢缝合。结果术后随访3—12个月,35例全部Ⅰ期愈合,效果满意。结论在鼻部亚单位组成的美容学基础,利用周围皮瓣可以修复鼻尖部的缺损,获得颜色、外形和纹理的一致,而且美学效果也令人满意。

Objective To introduce the experience on reconstruction of the skin defect of apex nasi after the tumor resection. Methods From February 2004 to February 2007, on basis of nasal aesthetics subunit principle, according to the size, shape and location of nasal defect and the conditions of surrounding skin, nasolabial flap was selected to cover the nasal defect in 35 cases which nasal tumors were removed. Among these eases, there were 21 men and 14 women, with age from 28 to 66 including 15 cases of basaloma, 9 eases of squamous cell eareinoma, 7 eases of melanin pigmented nevus and 5 cases of epidermal cyst. The clinical stage of malignant tumor was 0-I stage, with eases history from 3 months to 25 years, The area of nasal defect due to tumor resection ranged from 0.6cm×0.8cm to 1.5cm×2.0cm. The defect was directly repaired through suturring the nasolabial flap together. Results All 35 cases were available for follow-up of 3 to 12 months, All healed with the A grade and satisfied with efficeaey. Conclusion Based on the nasal aesthetics subunit principle, naselabial flap can reconstruct the defect of apex nasi, with a good color, contour and texture matched, and cosmetic results are also satisfactory.
