3种腭成形术对术后语音改善的比较

Comparison Study on the Three Palatoplasties by the Articulation Postoperated

目的在恢复腭咽部解剖形态和术后生理功能的方面,寻找到最佳手术方法。方法应用主观耳测评判和客观评定法,对75例腭裂患者术前术后的语音学特点进行评定。结果主观耳测评定方法结果显示:采用提肌重建术和软腭反向双"Z"瓣移位术,术后患者语音清晰度>70%的患者数量明显高于采用兰氏法,语音改善明显。结论提肌重建术和软腭反向双"Z"瓣移位术能够充分延长后退软腭,充分发挥腭帆提肌的作用,对实现完善的腭咽闭合具有重要意义。

Objective To find a best surgical operation in which can resume palatopharyngeal＇ s anatomical morphology and postpoperated physiological function. Methods By the listener and objective judgements, judge 75 palate patient＇ s velopharyngeal closure. Results The results of objective and subjective judgements shows： the articulation 〉 70% patient numbers with postoperated by Furlow double opposing Z - plasty and Levator muscle reconstruction has long higher than operated by Von langen beck, speech has distinct improvement, P 〈 0.05. Conclusion Furlow double opposing Z - plasty and Levator muscle reconstruction can increased velar length and play the effect of levator veil palati muscle, have an important significance for perfect VP.
