摘要
目的比较新型可调套式栓钉与拉力螺钉治疗Schatzker-Ⅰ型胫骨平台骨折的生物力学特性。方法选用成人胫骨标本6对,建立Schatzker-Ⅰ型胫骨平台骨折模型并分为两组,实验组应用新型可调套式栓钉进行固定,对照组使用普通拉力螺钉固定。分别检测两组模型在不同程度的垂直压力下(400、600、800 N)平台塑性沉降。结果两组模型在各不同程度压力作用下所测得的塑性沉降平均值比较均有统计学差异(P均<0.01)。实验组在各不同程度压力作用下的下陷距离与该组各标本骨密度值无相关关系,而B组呈明显负相关关系。结论新型可调套式栓钉是治疗Schatzker-Ⅰ型胫骨平台骨折的一种有效的新方法,对于骨质疏松患者能提供更好的帮助。
Objective To compare the biomechanical properties in the Schatzker-Ⅰ tibial plateau fracture model stabilized either by the new adjustable stud or by the lag screw. Methods A Schatzker- Ⅰ tibial plateau fracture was created in 6 pairs of adult human cadaver tibiae divided into 2 groups. Group A was stabilized by the new adjustable stud. Group B received the fixation by the lag screw. The plastic deflection was measured at three levels of vertical pressure (400 N,600 N, 800 N). Results There was significant difference between two fixation methods for average plastic deflection at each level of vertical pressure(P 〈0. 01 ). The bone mineral density showed no significant correlation to the vertical subsidence for group A while group B showed a significant negative correlation at two levels of pressure. Conclusion New adjustable stud is a new kind of method for the therapy of Schatzker-Ⅰ tibial plateau fracture and may be more helpful to the patients with osteoporosis.
