改良张力带治疗髌下结构损伤112例临床观察

A CLINICAL OBSERVATION OF IMPROVED TENSION BAND IN CURING THE DAMNIFICATION AT INFRAPATELLAR STRUCTURE OF 112 CASES

目的探讨髌下结构(髌韧带及其周围结构)损伤的新的治疗方法。方法损伤侧髌骨正中及胫骨结节处(相当于胫骨结节骨牵引水平)从外向内分别钻1枚克氏针,然后用钢丝在髌韧带前方"8"字固定两枚克氏针的末端,缝合、修补髌韧带及损伤的关节囊。结果术后3、6个月、1、2年随访,优良率分别为65.2%、92.0%、94.6%和97.3%,经比较,有非常显著性差异(P<0.01);但6个月、1、2年3组比较,无显著性差异(P>0.05)。结论改良张力带治疗髌下结构损伤较传统方法治疗有巨大的优越性,值得临床推广应用。

Objective To discuss a new method of curing the damnification at infrapatellar structure （patellar ligament and the structure around it）. Methods From two points,one at the damaging side of patella and the other at tibial tuberosity（the place used for traction） each drilled one Kirscher wire, and then fixed the end of Kirscher wire, stitched and repaired the patellar ligament and damaged articular capsule. Results Observed the follow-up surveys of 3 months, 6 months,1 year and 2 years after the operation,the cure rate were 63.2%, 92.0%, 94.6% and 97.3 %. The difference was significant in statistics （ P〈0.01） But there were no significant difference among 6 months, 1 year and 2 years group（ P 〉 0.05）. Conclusion Compared with traditional method, the improved tension band in curing the damnification at infrapatallar str.ucture had great advantages,so it was worth applying and spreading.
