细胞周期素D_1在中央型肺癌中的表达与其MSCT征象的相关性研究

摘要 目的:本研究将肺癌影像学表现同病理特征及cyclinD1蛋白表达相对照,探讨肺癌MSCT表现的分子病理学基础,从分子水平对肺癌的侵袭、转移、做出正确评估。方法:对58例经纤维支气管镜和病理活检确诊的中央型肺癌患者行MSCT平扫及双期动态增强扫描。将容积扫描数据传至AW4.2工作站进行三维重建及图像分析。所取标本病理切片进行免疫组织化学染色分析,测定不同组织类型及分化程度肺癌组织中的cyclinD1蛋白含量。统计学分析用SPSS10.0软件包进行数据处理。结果:58例中央型肺癌中cyclinD1阳性表达率为39.7%。高分化癌cyclinD1蛋白表达率分别为13.3%,低分化癌中表达率为48.8%,其结果有统计学意义(P<0.05)。淋巴结转移及远处侵袭36例,cyclinD1蛋白阳性表达率为52.8%,淋巴结转移组和非转移组相比有显著性差异(P<0.05)。结论:肺癌组织中cyclinD1蛋白的阳性表达同肿瘤分化程度及是否有淋巴结转移、远处侵袭密切相关,根据MSCT征象可推断出cyclinD1蛋白的表达情况。 Objective. Detecting the expression of eyelin D1 could help to understand the molecular pathologic base of MSCT features of lung carcer to plain and MSCT of lung cancer.Methods:Both plain MSCT scan and double phase dynamic contrast-enhanced MSCT scan were performed in 58 patients with central lung cancer. The scanning data were transferred to the workstation and post processed with multiple planar reformat (MPR) and volum rendering (VR). All images were analyzed retrospectively. The specimens were stained with HE and detection procedure for cyclin D1 were performed using immunohistochemistry. Statistical analysis was executed by SPSS 10.0 software. Results:In 58 patients, the expression of cyclin D1 was 39.7%. significantly related with tumor invasion and lymph node metastasis ( P 〈 0.05). In 58 patients, cyclinD1 protein expression was correlated with cell differentiation, invasive depth and lymphnode metastasis ( P 〈0.05). Conclusion: Cyclin D1 over expression may play an important role in the central lung cancer. They can he con sidered as a predicting marker of lung cancer. The combination of MSCT with cyclinD1 could help to evaluate the clinic-pathologic character of central lung cancer, the outside manifestations of the biological character of central lung cancer could he studied from the proteins level.
