良性前列腺增生MRS检查尿道周围单体波谱代谢特点研究

摘要 目的:探讨良性前列腺增生MRS检查尿道周围单体与不同病理类型前列腺组织的差异。方法:对16例前列腺增生的患者术前行MRS检查,观察枸橼酸盐(cit)胆碱化合物(cho)和肌酸(cre)波峰,并测定不同单体的(cho+cre)/cit值(包括尿道周围单体)。行耻骨上经膀胱前列腺摘除术后的标本进行大病理切片检查,将波谱图像的(cho+cre)/cit值与前列腺的病理类型分别进行记录。结果:前列腺腺体增生型(cho+cre)/cit值为0.59±0.29;基质增生型(cho+cre)/cit值为1.23±0.38;混合增生型(cho+cre)/cit值为0.80±0.27,尿道周围单体(cho+cre)/cit值为0.68±0.33。经统计处理,尿道周围单体与间质型增生及混合型增生存在统计学差异,与腺体型增生无统计学差异(P<0.05)。结论:尿道周围单体(cho+cre)/cit值主要取决于尿道周围的腺体组织,与腺体型增生组织的(cho+cre)/cit值相近,在前列腺MRS的诊断中具有重要的价值。 Objective: To study the characters of the unit through urethra in MILS of benign prostatic hyperplasia and the differences with three pathologic types. Methods:Magnetic resonance spectroscopy appearances were researched in sixteen patients with benign prostatic hyperpLasia, the peaks of citrate, choline and creatine were observed, the ratios of (choline± creatine)/citrate were measured including the unit through urethra. After suprapubic transvesical prostatectomy the specimens were inspected in the method of whole pathologic section. The values of (cho+ cre)/cit and pathologic types of benign prostatic hyperplasia were recorded correspondingly and handled statistically.ResuRs:The ratios of (choline ± creatine)/citrate of three pathologic types of benign prostatic hyperplas/a and the unit through urethra were 0.59 ± 0.29, 1.23 ± 0.38, 0.80 ± 0.27 and 0.68 ± 0.33 (mean± standard deviation). There were statistical differences among the unit through urethra, mixed hyperpLasia and interstitial hyperpLasia. There were not statistical difference between the units through urethra and glandular hyperpLasia( P 〈 0.05). Conclusion:The ratios of the unit around urethra depended on mainly glandular tissues near urethra. MILS may be useful to clear the characters of the units through urethra.
