功能性上颌窦手术与上颌窦根治术的临床比较

A CLINICAL COMPARISON BETWEEN THE FUNCTIONAL ENDOSCOPIC SINUS SURGERY AND RADICAL COLDWELLLUC OPERATION

目的为论证功能性上颌窦手术在临床的应用价值并探讨其与传统的上颌窦根治术的优劣。方法功能性上颌窦手术与传统的上颌窦根治术治疗共６８例上颌窦疾病患者，进行术后随访。其中功能性上颌窦手术（Ａ组）３２例（４６侧），传统上颌窦根治术（Ｂ组）３６例（４２侧），均作了鼻及鼻内窥镜检查。结果Ａ组的治愈率及造口通畅率均较Ｂ组为高，但窦腔及窦口息肉以Ａ组发生率为高。结论前者疗效较后者更具优势，但亦存在不足，并不能全面取代传统的上颌窦根治术。

Objective To confirm the clinical value of the functional endoscopic sinus surgery with a comparison to radical ColdwellLuc operation.Methods 68 patients including 32 cases (46 sides) who had been performed functional sinus surgery, e.g. an operation by enlarging natural maxillary ostium,and 36 cases(42 sides)performed by radical ColdwellLuc operation were followed up and examined by endoscope.Results[WT5”BZ] The cure rate and degree of unobstruction in the group that received functional operations were better than those received ColdwellLuc operations,but the adverse effects of antral polypus and cysts were found more in the former group.Conclusion Though the curative rate of functional endoscopic sinus sergery was better, radical ColdwellLuc operation could not be replaced yet, due to more defects found in the former.
