脑电图对CT尚未显影的大面积脑梗死定位诊断与意义 被引量:2

摘要 目的进一步探讨脑电图(EEG)与脑电地形图(BEAM)在CT尚未显影的大面积脑梗死急性期的辅助诊断价值。方法33例大面积脑梗死急性期患者,CT尚未显影,住院24h内行床旁脑电图检查,选择理想信号段进行功率谱分析,住院后1~4d复查CT证实为大面积脑梗死。将CT与脑电图进行对比研究。结果脑电图异常率100%,功率谱分析显示患侧α频带功率值降低,慢波频段功率值增高。BEAM定侧与头CT基本相符。结论脑电图检查对CT尚未显影的大面积脑梗死具有辅助诊断价值,BEAM的α频带与慢波频带功率值的局限性改变有助于定侧,综合考虑α、θ、δ波功率的变化会提高定侧的准确率。 Objective To explore the value of EEG and BEAM in the assistant diagnosis of acute massive cerebral infarction with negative CT. Methods Electroencephalograph was examined in 33 patients with acute large cerebral infaction in 24 hours after hospitalization. The result of cranial CT was negative. The selected data were analyzed to get the power spectrum. After 1 to 4 days of hospitalization,massive hemispheric infarction was apparent on CT, and compared CT with EEG. Results The abnormality rate of EEG in the patients was 100% ,power spectrum revealed that α power values of the lateral decreased and slow wave power values increased. The diagnostic infarction lateral on BEAN was consistent with crianal CT. Conclusion EEG might be a useful tool for diagnosis of massive cerebral infarction with negative CT. The focal alteration in power of α and slow wave frequency band in BEAN may be help to lateralize. Combined with the focal alteration in power of α,θ and δ may improve the accuracy in diagnostic infarction lateral.
