新生儿听力筛查假阳性的原因分析及护理

THE CAUSE ANALYSIS AND NURSING STRATEGY OF THE FALSE POSITIVE RESULTIN AUDITORY SCREENING OF NEONATE

目的探讨新生儿听力筛查假阳性的原因及护理对策。方法应用AuDx型(DPOAE)耳声发射分析仪对2006年8月～2007年8月在我院产科出生的350例新生儿进行测听分析,对其中假阳性病例的原因进行分析并采取相应护理对策。结果13例连续3次未通过,为DPOAE测试阳性,阳性率为3.71%;后经ABR测试检查确诊1例听力异常,假阳性率为3.42%,假阳性的原因有耳内耵聍或分泌物阻塞、婴儿状态欠佳、耳塞的选择与放置不当等。结论多种因素可导致新生儿听力筛查假阳性,采取清除耳道分泌物、选择合式的测听时机、选择合适耳塞等护理对策,可使新生儿听力筛查的假阳性率显著降低。

Objective To investigate the cause and nursing strategy of the false positive result in auditory screening of neonate. Methods Auditory screening was performed in three hundred and fifty neonates in our department from Aug. 2006 to Aug. 2007. The causes of false positive were studied, and the according nursing strategy was performed. Results There were 13 neonates who haven＇ t passed the test, positive rate was 3.71% ; and the final diagnosis of dysaudia was made in 1 neonate, the false positive rate was 3.42%. The cause of false positive was cerumen clogging, the malcondition of neonates and the unsuitable selection and placing of earplug, etc. Conclusion Many factors influenced the false positive in auditory screening of neonate, a set of nursing intervention including cleaning the ear wax, select the suitable opportunity of test and the suitable ear plug, can cut down the false positive rate.
