窄谱和宽谱中波紫外线对银屑病患者血清IFN-,IL-4影响的研究

摘要 目的研究311nm窄谱中波紫外线(NB-UVB)与280-320nm宽谱中波紫外线(BB-UVB)对寻常型银屑病患者血清IFN-γ和IL-4的影响。方法60例寻常型银屑病患者随机分为NB-UVB、BB-UVB、对照3组,每组20例,3组患者除均用VitC、钙剂、海棠合剂以及外用松榴油治疗外,前两组每周光疗3次,共4周,对照组不予光疗。治疗前后分别以银屑病严重程度PASI评分标准评分,以ELISA法测定血清中IFN-γ和IL-4,并与10例正常人血清比较。结果治疗前3组PASI评分无差异(P〉0.05),治疗后3组PASI评分均下降(P〈0.05),且NB-UVB组低于BB-UVB组,BB-UVB组低于对照组;3组治疗前血清IFN-γ水平较正常人高(P〈0.05),但IL-4和正常无明显差异(P〉0.05),IFN-γ/IL-4水平较正常人高(P〈0.05);治疗后3组血清IFN-γ水平均下降,IL-4水平无明显变化,IFN-γ/IL-4水平降低(P〈0.05),但下降程度不同,NB-UVB组低于BB-UVB组,BB-UVB组低于对照组。结论NB-UVB治疗寻常型银屑病在临床改善程度和血清IFN-γ/IL-4水平下降水平方面均优于BB-UVB。
