改良括约肌保留术治疗高位肛瘘临床观察

Clinical Observation on the Therapeutic Outcome of the Improved Procedure of Anal Sphincter Preservation in the Treatment of High Anal Fistula

为了提高治疗高位肛瘘的手术效果，采用肛门外切口间断全层缝合，肛门内切口开放引流，保留括约肌治疗高位肛瘘40例。治愈率95％，疗程14～35d，随访1～3年，无复发和后遗症。结果表明，该术式不仅切除了原发病灶及部分瘘道，而且保留了肛门括约肌，不损伤肛门功能。与传统切开挂线术比较，具有痛苦小、恢复快、不易复发等优点。

Forty patients with high anal fistula underwent the surgical procedure of interrupted all-layer suture for the outer anal incision, open drainage for the inner anal incision and anal sphincter preservation in an attempt to improve the surgical efficacy. As results, the treatment course was 14-35 days, the cure rate was 95.0% ,and follow Up of 1-3 years showed no recurrence and sequellae. In this procedure,the primary focus and partial fistula were resected,anal sphincter was preserved,and the anal function was intact. It is concluded that this procedure is better in terms of less pain,quick recovery and less recurrence among other strong points,as compared with traditional procedure.
