煤工尘肺并肺性脑病与多系统器官功能障碍临床分析

Clinical analysis of multiple system organ failure in pneumonoconiosis of coal industrial labor with pulmonary encephalopathy

摘要 目的探讨煤工尘肺并肺性脑病与多系统器官功能障碍的关系。方法选择煤工尘肺并肺性脑病患者34例,将其伴发的功能障碍器官个数与病死率作相关性分析,并同时测定其酸碱度及电解质水平。结果煤工尘肺并肺性脑病100%并发器官功能障碍,最常受损器官:肺、心、脑、肝或肾,受损器官数目越多病情越严重,并且酸碱失衡(呼酸代酸)及电解质紊乱(低钠低氯)也相应增多。结论100%煤工尘肺并肺性脑病患者伴发多器官功能障碍,随功能障碍器官数目的增加病死率亦增加,酸碱失衡及电解质紊乱与多器官功能障碍有关。 Objective To investigate the relationship between the pneumonoconiosis of coal industrial labor and pulmonary encephalopathy. Methods Thirty - four patients with pneumonoconiosis and pulmonary encephal opathy were selected. The number of failure organ and the death rate were analysized for the correlation,meanwhile;the serum pH and the level of electrolytes were detected in all patiens. Results There were organ failure in all the patients with pneumonoconiosis and pulmonary encephalopathy, the most damaged organs,heart,brain, pulmonary, liver or nephro. More organs damaged, the severer the disease was acid -base imbalance (respiratory acidosis, metabolicacidosis) and electrolytes disturbances ( hyponatremia, hypochloraemia) obviously accordingly. Conclusion Multiple system organ failure and pulmonary encephalopathy accompany in 100 percent patiens of pneumonoconiosis ,with the number of organ failure increasing the death rate also raised acid -base imbalance and electrolytes disturbance is correlated with multiple system organ failure.
