血浆血管内皮生长因子和尿微量白蛋白及β2-微球蛋白与过敏性紫癜患儿早期肾损伤的相关性研究 被引量:7

摘要 目的探讨血浆血管内皮生长因子(VEGF)、尿微量白蛋白(MA)及β2-微球蛋白(β2-MG)与过敏性癜(HSP)及过敏性紫癜肾炎(HSPN)发病关系;探寻HSP早期肾损伤敏感的实验室指标。方法实验分组为:过敏性紫癜组,根据送标本时临床诊断标准分组为:皮肤型组、混合型组、HSPN型组;健康对照组;治疗后组。本实验测定血浆VEGF采用ABC—ELISA法,测定尿MA及β2-MG采用放射免疫分析法。所有统计资料采用SPSS12.0统计分析软件包进行分析,以P〈0.05为差异有统计学意义。结果HSP早期各临床分型比较发现HSPN型组血浆VEGF、尿MA及β2-MG水平分别高于其他临床型,差异有统计学意义(P〈0.05);治疗组高于对照组(P〈0.05),差异有统计学意义。血浆VEGF、尿MA及β2-MG阳性率分别与其他指标阳性率相比较差异有统计学意义(P〈0.05)。结论HSP早期患儿血浆VEGF、尿MA及β2-MG水平均明显增高,伴有肾损伤者升高尤为明显,恢复期有所降低但仍处于较高水平。本研究结果表明血浆VEGF、尿MA及β2-MG是早期HSP并发肾损伤的敏感指标。 Objective To detect and explore the changes of plasma vascular endothelial growth factor (VEGF) urine microalbumin(MA) and β2 - Microglobulin( β2 - MG) in children with henoch - schonlein purpura (HSP). Methods Children involved in the experiments were divided into three groups: HSP group: HSP were divided into pure purpura group, mixed pattern group and HSPN group; normal control group; recovery stage group. The experiment was double - antibody sandwich ABC - ELISA detected VEGF, detected MA and β2 - MG by radioimmunoassay. Results HSP comparison of clinical classification found that the plasma VEGF, urine MA and β2 - MG concentration at acute period of renal involvement group compared with others groups show statistical significance ( P 〈 0. 05 ). After - treated groups was higher than that in control group( P 〈 0. 05). In the early period of 73 HSP children, high β2 -MG 49 cases (67.12%) ,more VEGF 51 cases (69.86%), MA 53 cases (72.60%)highest. VEGF than other index( P 〈0.01 ). Conclusion The average levels of plasma VEGF, urine MA and β2 - MG are increased significantly particularly with early renal damage in HSP patients. Plasma VEGF, urine MA and β2 - MG are sensitive markers of renal damage in early stage of HSP patients.
