急性肾衰竭预后及其影响因素的研究

Research on prognosis and influencing factors of acute renal failur

目的探讨急性肾衰竭（ARF）预后及影响因素。方法回顾性分析1998年1月至2006年12月收治的398例ARF患者的临床资料，根据患者的病因、年龄、有无基础疾病、是否行肾脏替代治疗、是否合并多脏器功能衰竭分析ARF的预后。结果并发严重心脑血管疾病、严重感染、恶性肿瘤者预后较差；有基础疾病、老年、未行肾脏替代治疗、合并其他脏器功能衰竭者预后差，与对照组比较，差异有统计学意义（P〈0．05）。结论ARF的预后受多种因素的影响，临床医师应予以充分透析治疗，防治并发症，以改善预后。

Objective To explore the prognosis and influencing factors of acute renal failure. Methods The clinical data of 398 patients with ARF in our department from January 1998 to December 2006 was analyzed, According to： etiology,age,with or without fundamental diseases, dialysis and non-dialysis, complicated with or without multiple organ failure, the prognosxis and influencing factors of ARF were explored. Results ARF complilating to serious cardiovascular and cerebrovascular diseases, severe trauma, infection, cancer, were associated with poor prognosis, patients with fundamentd diseases, old age, without renal replacement therapy, developed multiple organ failure also with poor prognosis, there was significant difference compared with control group （P〈0. 01 ）. Conclusions Many factors can affect the prognosis of ARF, clinicians should raise awareness of the prognosis and affected factors of ARF. Initiation of early dialysis and prevention of complication, may improve the prognosis of ARF.
