软读片在细微骨折诊断中的应用

The Role of Soft Reading in Diagnosis of Fine Bone Fracture

目的:探讨软读片在急诊外伤患者细微骨折诊断中的作用。材料与方法:在本院PACS服务器上随机调取2005年2月～2006年5月1256例急诊外伤病人的X线图像。直接在显示屏上读片,并使用图像处理软件调节图像,初步筛选出可供分析的细微骨折病例73例,进行回顾性统计分析。结果:可供分析的细微骨折病例73例中,经过随访确诊的细微骨折28例,1例排除细微骨折。其中读片医师漏诊18例,占总病例的62.07%。所有病例的诊断中,核片医师认为放大或调节对比度起了作用的26例(89.66%),未起作用的3例(10.34%),经过X2检验,P<0.05。结论:应用显示屏软读片,可提高阳性X线征象的显示率,特别是对细微骨折的显示更为重要。尤其是肋骨细微骨折,具有较大的临床意义。

Purpose： To explore the role of soft reading in diagnosis of fine bone fracture of traumatic patient in emergency. Materials and Methods： To collect the materials of traumatic patient in emergency from february 2005 to May 2006 on PACS servers, which was 1256 cases. To read the radiograph on the monitor directly, with adjusting the radiograph in the image - managing software, from which we preliminarily screen out 73 fine bone fracture cases that can be analyzed retrospectively. Results： Under follow up, among the 73 cases, the diagnosis of fine bone fracture were certain in 28 cases, 1 case were excluded. of them 18 cases were missed by reading - radiography doctors, which was 62.07 %.Among all cases, soft reading were regarded as a important role in 26 cases （89.66 % ） by examining-radiograph doctor, no important in 3 cases （ 10.34）, P 〈 0.05 under X^2 test. Conclusion： The rate ot showing positive X - ray feature could be improved by using soft reading on monitor, especially in showing fine bone fractur be very important in showing fine rib bone fracture.This word should be very important in showing fine rib bone fracture in clinical.
