心力衰竭患者外周血淋巴细胞与心肌组织中钙调神经磷酸酶信使核糖核酸水平相关性研究的初步报告 被引量:4

摘要 目的:研究心力衰竭患者外周血淋巴细胞与心肌组织中钙调神经磷酸酶(CaN)信使核糖核酸(mRNA)的相关性。方法:选择2007-01至2007-08因二尖瓣狭窄伴关闭不全接受二尖瓣替换术的心力衰竭患者38例(心功能Ⅱ级11例、Ⅲ级15例、Ⅳ级12例),正常对照12例,彩色多普勒超声心动图测量心功能参数。取静脉血和少量左心室前乳头肌,分离静脉血淋巴细胞后,提取淋巴细胞及乳头肌组织的总RNA,用半定量逆转录多聚酶链反应技术分别测定CaN A亚基α、β异构体(CnAα和CnAβ)mRNA在两者中的水平。结果:38例慢性心力衰竭患者外周血淋巴细胞和心肌组织中的CnAαmRNA水平呈正相关r=0.820,P<0.01;CnAβmRNA水平呈正相关r=0.875,P<0.01。心功能Ⅱ级、Ⅲ级、Ⅳ级患者外周血淋巴细胞和心肌组织的CnAα和CnAβmRNA水平较正常对照者升高,差异有统计学意义(P<0.05~0.01),且随心力衰竭级别增加,CnAβmRNA增加更显著,差异有统计学意义(P<0.01)。外周血淋巴细胞CnAβmRNA水平和心功能级别呈正相关r=0.877,P<0.01;心肌组织CnAβmRNA水平和心功能级别呈正相关r=0.805,P<0.01。结论:外周血淋巴细胞CaNA mRNA水平可以反映心肌组织中的CaNA mRNA水平。 Objective : To study the relationship of calcineurin mRNA level between peripheral lymphocytes and cardiac muscles of patients with chronic heart failure. Methods: Thirty eight congestive heart failure patients with rheumatic mitral valve disease were selected and twelve cases of healthy persons were included as controls. Cardiac function parameters were measured by echocardiography. The mRNA level of CnAα and CnAβ in left ventricular anterior papillary muscle and peripheral lymphocytes was determined by semi-quantitative reverse transcription polymerase chain reaction. Results: CaN mRNA level in lymphocyte was positively correlated with that in cardiac muscle( CnAα mRNA r = 0. 820, P 〈 0. 01 ;CnAβ mRNA r =0. 875 ,P 〈0. 01 ). Compared with the normal persons, CnAα and CnAβ mRNA level in circulating lymphocytes and cardiac muscle was higher in heart failure patients. CnAβ mRNA level was upregulated significantly with cardiac function class classified by New York Heart Association (peripheral lymphocytes r = 0. 877,P 〈 0. 01 ;cardiac muscle r = 0. 805,P 〈 0. 01 ). Conclusion: The peripheral lymphocyte CaN mRNA level can represent the level of cardiac tissue.
