摘要
目的:利用MDCT血管造影评价膈下动脉,为拟行动脉导管栓塞术的肝癌患者术前提供更多信息。材料和方法:对128例经MDCT增强扫描的肝癌患者,于动脉期行智能跟踪法扫描,记录双侧膈下动脉的起源、大体解剖形态,了解是否参与肿瘤供血。结果:128例可见右膈下动脉起源及大体解剖形态,126例可见左膈下动脉起源,双侧膈下动脉按起源可分为8型,以起源于腹主动脉及腹腔干为主,双侧IPA可共干或分别起源,其中右膈下动脉参与肝癌供血5例。结论:MDCT血管造影能清晰显示膈下动脉的起源、解剖形态,为肝癌患者动脉导管栓塞术前提供更多信息。
Purpose: To evaluate the inferior phrenic arteries (IPA) with angiography of multi-detector row computed tomography (MDCT) , providing more information for patients with hepatic carcinoma before transcatheter arterial chemoembolization. Materials and Methods: A total of 128 patients with hepatic carcinoma underwent MDCT contrast-enhanced scanning were enrolled in this study, Smartprep technique were used in arterial phase and the origins and the anatomic shape of bilateral IPA were noted to find out whether they participated in the blood supply of hepatic carcinomas. Results: The origins and shape of the right inferior phrenic arteries (RIPA) were clearly visualized in 128 cases, and the origins of the left inferior phrenic arteries (LIPA) were clearly visualized in 126 cases. The origins of bilateral IPA could be divided into 8 types with the main origins from abdominal aorta and celiac trunk, and bilateral IPA could originate from a common trunk or separately. There were 5 cases with RIPA participated in the blood supply of hepatic carcinomas. Conclusion : Angiography technique of MDCT can provide more information for patients with hepatic carcinoma before transcatheter arterial chemoembolization with clearly visualization of the origin and shape of IPA.
