股骨重建髓内钉治疗股骨粗隆间骨折的临床应用

Treatment of Intertrochanteric Femoral Fracture with Recon struction Femoral Interlocking Nail

目的探讨股骨重建髓内钉治疗股骨粗隆间骨折的临床疗效。方法回顾性分析在2000年1月至2006年1月收治股骨粗隆间骨折30例,采用Evan’s分型,其中Ⅰ型6例,Ⅱ型5例,Ⅲ型12例,Ⅳ型7例。以Russell-Taylor股骨重建髓内钉静力型固定。结果本组28例患者获得随访,2例失访。其中2例Ⅳ型骨折出现轻度髋关节内翻,髋关节外展轻度受限;其余患者均无髋内翻,股骨颈干角正常,骨折对位对线良好。结论对股骨粗隆间骨折的治疗,Russell-Taylor股骨重建髓内钉静力型固定符合生物力学,尤其对Evan’sⅢ、Ⅳ型骨折有独特的优势,具有手术创伤小、失血少、固定可靠等优点。

Objective This article reported the effection of the application of reconstruction femoral interlocking nail on the intertrochanteric femoral fracture.Methods Thirty cases of the traumatic fractures were treated with the nail.The fractures were classified according to Evan＇s type：the type Ⅰin 6 cases,type Ⅱin 5 cases,type Ⅲ in 12 cases,type Ⅳ in 7 cases.Results 28 cases were followed-up,there were no severe complications,such as the infection and breakage,the function was good,except 2 cases suffered mild coax vara after operation.Conclusion The reconstruction femoral interlocking nail is the best way for the treatment of the intertrochanteric femoral fracture,espescily for the type Ⅲ and Ⅳ cases,as the reconstruction femoral interlocking nail according to the femoral biomechanics.
