主动脉弓离断畸形的超声心动图诊断

Echocardiographic diagnosis of interrupted aortic arch

目的探讨超声心动图对主动脉弓离断畸形早期诊断的价值和意义。方法回顾性分析19例主动脉弓离断畸形患儿的超声心动图检查资料,并与心导管造影检查和手术结果相对照。结果与心导管造影和手术结果相对照,超声心动图正确诊断17例,漏诊2例。确诊病例中病理分型基本正确,其中A型14例,B型2例,C型1例。漏诊病例中1例为A型,另一例为B型。19例患儿均合并动脉导管未闭和室间隔缺损,其中1例合并完全性肺静脉异位引流,2例合并房间隔缺损。结论超声心动图可早期诊断主动脉弓离断畸形,其操作简单易行,既无创又有较强的重复性,对提高该病早期确诊率具有重要的临床意义。

Objective To explore the value and the significance of echocardiography in diagnosing interrupted aortic arch. Methods Nineteen cases of interrupted aortic were analysed by echocardiography. The Cardiac catheter angiocardiography and conventional operations were used as the gold standards. Results Compared to the results of the cardiac catheter anglocardiography and conventional operations, 17 patients were diagnosed and classified correctly by eehoeardiography （ 14 type A, 2 type B and 1 type C ） and two patients false negative diagnosis （ 1 type A and one type B）. All the patients were complicated with congenital cardiovascular disease： ventricular septal defect and patent ductus arteriosus （ 1 case with total anomalous pulmonary venous drainage and 2 cases with atrial septal defect）. Conclusions Echocardiography provides correct diagnosis of interrupted aortic arch due to its non-invaslon, safety, convenience, facility and reproduction, which is very important for early and correct diagnosis of this disease.
