摘要 目的探讨BiPAP呼吸机在心脏术后发生急性心功能不全时的治疗价值。方法对22例心脏术后发生急性心功能不全的患者,在常规抗心衰治疗的基础上加用BiPAP呼吸机支持,同时进行动脉血气、心率、呼吸频率等生命体征的监测和分析。结果在使用BiPAP呼吸机后,所有患者氧合情况和低心排症状都有明显改善,PaO2,SaO2明显上升(P<0.01),心源性肺水肿减轻,心率血压恢复正常。结论心脏术后出现急性心功能不全伴低氧的患者,在常规抗心衰治疗的基础上,早期应用BiPAP能迅速纠治低氧血症、改善心功能,是一种安全有效的治疗方法。 Objective To evaluate the efficiency of bi-level positive airway pressure (BiPAP) respirator in managing acute cardiac failure after cardiac surgery. Methods Twenty-two patients with acute cardiac failure after cardiac surgery were treated with BiPAP respirator in addition to conventional treatment. Changes of clinical symptoms such as arterial blood gas, heart rate (HR) and respiratory frequency were detected and analyzed. Results The clinical symptoms of 22 patients had been improved remarkably after using BiPAP respirator. All the clinical parameters such as PaO2, SaO2, HR improved significantly. Conclusion In early stage, using BiPAP respirator on the basis of connentional anti-heart failure treatment can quickly relieve hypoxemia and improve cardiac function. It is a safe and effective method for patients with acute heart failure after cardiac surgery.
