心脏再同步化治疗慢性心力衰竭临床疗效观察

Clinical Observation of Cardiac Resynchronization Therapy for Patients with Chronic Heart Failure

目的观察心脏再同步化治疗对慢性心力衰竭患者的临床疗效.方法观察慢性心衰患者行心脏再同步化治疗临床指标,包括纽约心功能分级(NYHA)、心电图QRS时限、心脏彩超左心室舒张末期内径(LVEDD)、左心室射血分数(LVEF)、运动耐量(6 min步行距离)等指标.结果22例患者死亡,其中由于心衰恶化死亡13例,心源性猝死7例,电击死亡1例,脑卒中死亡1例,4名患者失访,21例患者存活,其中行三腔心脏起搏加体内自动转复除颤的患者全部存活,存活患者心功能及运动耐量有明显提高.结论CRT能改善心衰患者的生存,CRT/D能降低心衰患者的猝死。

Objective To observe the clinical effect of Cardiac Resynchronization Therapy （CRT）on patients with chronic heart failure （CHF）. Methods We observed clinical indicators of 47 patients with CHF implanted CRT,including NYHA, duration of QRS, LVEDD, LVEF and walk distance during 6 minute. Results 22 patients died,4 patients lost communication,21 patients survived. The NYHA class and LVEF of the live patients was improved. Conclusions CRT is an effective treatment for chronic heart failure,it can improve the quality of lives of CHF patients and decrease the rate of sudden cardiac death.
