摘要
目的探讨TNFα-、caspase-3、MMP-2与MMP-9在子痫前期胎盘的表达及其临床意义。方法选取正常妊娠(对照组)、轻度子痫前期(实验组1)以及重度子痫前期(实验组2)各10例,采用免疫组织化学法检测TNFα-、caspase-3、MMP-2与MMP-9在胎盘的表达,并采用Morphology/BX51系统进行图像分析。结果TNFα-、caspase-3、MMP-2和MMP-9的阳性细胞主要为细胞滋养细胞与合体滋养细胞,但表达趋势不同,TNFα-与caspase-3在对照组、实验1组和实验2组组织的积分光密度和阳性细胞数量逐渐增高,MMP-2和MMP-9在各组的表达则逐渐减少(P<0.01)。相关分析表明,TNFα-与cas-pase-3的表达量呈正相关,与MMP-2和MMP-9的表达量呈负相关。结论TNFα-在子痫前期胎盘组织的表达增加可能是子痫前期发病的重要环节,与患者外周循环TNFα-的增高、胎盘细胞滋养细胞凋亡和浸润能力下降有关。
Objective To explore expression of TNF-α,caspase-3,matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)in placenta in pre-eclampsia.Methods Ten cases of normal pregnancy,10 cases of mild pre-eclampsia and 10 cases of severe pre-celampsia were chosen and divided into control group,experiment group 1 and experiment group 2,respectively.Levels of TNF-α,caspase-3,MMP-2 and MMP-9 in placenta were detected by immunohistochemical technique and analyzed by M-orphology/BX51 photo analysis system. Results TNF-α,caspase-3,MMP-2 and MMP-9 were mainly expressed in cytotrophoblasts and synthytiotrophoblasts.In control group,experiment group 1 and group 2,levels of TNF-α and caspase-3 gradually increased(P〈0.01),but levels of MMP-2 and MMP-9 decreased(P〈0.01).TNF-α expression was positively correlated to caspase-3 expression,but negatively correlated to expression of MMP-2 and MMP-9.Conclusion Elevated levels of TNF-α in placenta may play an important role in pathogenesis of pre-eclampsia,which may be associated with the increase of TNF-α expression in blood,enhanced apoptosis and decreased invasive ability of trophoblast cells in pre-eclampsia.
