治疗性乙肝疫苗免疫效果观察

Immune Effect of Therapeutic Hepatitis B Vaccine

目的观察治疗性乙肝疫苗的血清学免疫效果。方法观察对象为60例慢性乙肝病人和HBV携带者,分为3组,A组:HBeAg阳性,ALT在正常值2倍以上的肝炎病人,经抗病毒和保肝治疗至ALT正常后注射治疗性乙肝疫苗(重组蛋白疫苗);B组:抗-HBe阳性者;C组:单纯HBsAg阳性者。ABC3组均每月注射1次,连续注射6次,3组均在第9个月时检验HBV血清标志物5项。结果经过6次以上的治疗性乙肝疫苗的治疗,A组HBsAg无1例转阴,血清学转换率为46.15%,B组的HBsAg转阴率为42.66%(16/38),C组为66.67%(6/9),总体HBsAg的转阴率达31.67%。结论治疗性乙肝疫苗是一种提高自身特异性免疫力的生物制品,在乙肝病毒的血清学转换和转阴方面有着良好的治疗效果,值得推广。

[ Objective ] To observe the serological immune effect of hepatitis B vaccine. [ Methods ] 60 cases of patients with chronic hepatitis B and HBV carriers were divided into three groups, Group A ： HBeAg positive, hepatitis patients with ALT 2 times greater than the normal, Patients with normal ALT received injection treatment of hepatitis B vaccine after the liver protection and anti-virus treatment （recombinant protein vaccine） ; [ Results] After more than six times treatment of therapeutic hepatitis B vaccine, serological conversion rate among group A was 46. 15%, and no one was HBsAg negative . HBsAg negative rate among group B and group C were 42.66% （ 16/38 ）66.67% （ 6/9） respectively, and total HBsAg negative rate accounted for 31.67%. [ Conclusion ] Therapeutic hepatitis B vaccine is a kind of biological products which can enhance self-specific immunity, and have good effect on hepatitis B serological conversion and HBsAg negativity, so it is suitable for promotion.
