婚姻支持对精神分裂症患者病情及生活质量的影响

Effect of Marital Supports on Quality of Life and Pathogenetic Condition in Schizophrenic Patients

目的探讨婚姻支持对临床治愈精神分裂症患者病情和生活质量的影响。方法采用Olson婚姻质量问卷、兰开夏生活质量概况表(LQOL)和简明精神病评定量表(BPRS)对206例已婚精神分裂症患者的婚姻质量、生活质量、精神症状严重程度进行评定。结果高婚姻质量组(婚姻质量评分≥332.56分,88例)BPRS总分及各因子分显著低于低婚姻质量组(婚姻质量评分<332.56分,118例;均P<0.01),服药完全依从性显著高于低婚姻质量组(P<0.01),LQOL(除居住环境、法律安全因子外)各维度评分显著高于低婚姻质量组(均P<0.01);Olson婚姻质量问卷中婚姻满意度、性格相容性、夫妻交流、解决冲突方式、性生活、与亲友的关系、角色平等因子分和Olson总分与LQOL总体生活状况分呈显著正相关(均P<0.01)。结论婚姻支持对维持和改善精神分裂症患者的病情和生活质量有直接关系,医护人员应注意引导患者和配偶增强主动寻求和利用婚姻支持的意识,以提高患者的康复效果和生活质量。

Objective To explore the effect of marital supports on quality of life and pathogenetic condition in schizo- phrenic patients. Methods Two hundred and six married schizophrenic patients were evaluated with Olson Marital Quality Questionnaire, Lencashire Quality of Life Profile （LQOL） and Brief Psychiartic Rating Scale （BPRS）. Results In high marital quality group （score of marrital quality was equal or more than 332.56）, the total score and the score of each items of BPRS were significantly lower than those in low marital quality group （P〈0.01 for all）, the compliance with drug was significantly higher （P〈0.01）, and the each dimension score of LQOL （except for inhabited environment and law safety） were significantly higher than those in low marital quality group （P〈0.01 for all）. The total score of quality of life was positively correlated with satisfaction of marriage, compatibility of character, inter-couple communication, conflict-solving style, sex life, relationshiip with relatives, roles equality as well as total scores of Olson Marital Quality （P〈0.01 for all）. Conclusion The marital supports are very important for improving the quality of life and pathogenetic condition in schizophrenic patients. The staff should guide patients and their spouses to strengthen the consciousness of initiative exploration, reasonable utilization of marital supports in order to improve rehabilitation efficacy and quality of life.
