改良外眦锚着术行下睑成形和中面部提升

The modification procedure of outer canthus anchor fix blepharoplasty and middle facelifting

目的:探讨改良外眦锚着术式行下睑成形和中面部提升临床手术方法和适应证的选择及并发症的防治。方法:经传统睑袋切口向外眦角正中纹延长切口5mm,按设计切除多余皮肤,分离下睑眼轮匝肌瓣,将分离好的轮匝肌瓣外侧头切断去除3～5mm使之缩短,将肌瓣外侧端与外眦韧带由内向外U字形缝合锚着固定,同时通过外眦角睑缘切口向下分离颧韧带并向外上方同样缝合锚着固定在外眦韧带及其周围骨膜上提升中面部,所有病例均不切开眶隔,不去除眶隔内脂肪。结果:术后均一期愈合,下睑外观满意,泪沟和鼻唇沟加深畸形消失,随访1～6年,效果满意。结论:外眦锚着术是为了确保眼睑在外眦部牢固附着而采用的一类基本技术,同时可经下睑切口行中面部提升术。

Objective To report the author＇s method of outer canthus region＂anchor-fixation technique＂for blephroplasty and facelifting. Methods To make the traditional blephroplasty in-cision, remove the bagginess, fatty protrusions, and lax hanging skin, cut the outer head of musculus orbicularis oculi for 3-5mm, U-shape suture anchor fixation is to be made. Conclusion The technique can make the fixation of orbicular muscle tighter. It will be easier to make facelifting with blephroplasty simutaneously.
