牙髓钙化形成的原因探讨

Analysis of the causes of the pulp calcification formation.

目的:探讨牙髓钙化形成的过程和原因。方法:显微镜下观察正常牙齿、牙髓炎患牙和外伤牙牙髓组织的牙髓钙化形成情况,推测牙髓钙化形成的机制。结果:30例正常牙齿牙髓未见有牙髓钙化发生,也未见异常组织学变化。25例牙髓炎患牙牙髓组织中8例(32%)有髓石形成。牙髓组织有典型炎症细胞浸润区域,有些部位形成化脓灶,化脓区周边为牙髓充血区。在炎症区和充血区之间有髓石形成。25例牙髓炎中有2例出现髓腔内钙化,11例根管内牙髓充血明显,发现3例(12%)根髓弥散性钙化。20例外伤牙牙髓组织中,髓腔和根管内牙髓均充血,未见髓石形成,有3例(15%)根管内牙髓出现弥散性钙化。结论:牙髓钙化的形成与牙髓感染、炎症刺激和血管充血密切相关。髓石形成与牙髓感染或牙髓炎有密切关系。根管弥散性钙化与牙外伤或牙髓充血有密切关系。牙髓钙化可能是一种主动性修复过程,而非被动的牙髓组织变性。冠髓髓石和根髓弥散性钙化的性质可能不同,两者的形成机制可能不一致。

Objective： Try to find out the causes of the pulp calcification formation. Method： To search for pulp stone and.channel diffused calcification through observing pulp tissues of 30 normal pulp tissues 25 pulp inflammatory tissues and 20 traumatic pulp tissues under microscope.To infer the causes of the pulp stone and diffused calcification formation from relevant conditions under microscope. Result： There are no pulp calcification formation in 30 normal pulp tissues. There are typical inflammatory cell infiltration and blood vessel congestion areas in 25 pulp inflammatory tissues. There are 8 cases （32.0%） with pulp stones formation between inflammatory cell infiltration and blood vessel congestion areas and there 3 cases with channel pulp diffused calcification formation There are 3 cases （15.0%）with diffused calcification in the traumatic channel pulp tissues and no pulp stone formation in the traumatic teeth pulp chamber. Conclusion： Pulp stone formation is closely related to inflammation of the pulp. The tooth channel diffused calcification is closely related to traumatic pulp. It may be a positive repairing course but a passive tissue degeneration. The formation mechanism of the pulp stone may be different from the channel calcification.
