摘要
目的观察乙酸灌注兔肝门静脉分支后肝脏的声像图、病理及生化改变,探讨乙酸栓塞门静脉分支引起兔肝脏坏死的机制。方法28只兔剖腹后经门静脉分支注入不同剂量的50%乙酸。按灌注乙酸的量0.250,0.188,0.125,0.050,0.025ml/kg分为5组。结果乙酸灌注兔肝门静脉分支后,可造成门静脉分支管腔内膜炎症,管腔狭窄,甚至血栓形成,伴行的肝动脉分支也因乙酸作用形成内膜炎及血栓,导致相应区域肝组织变性坏死,肝肾功能指标随之变化。结论经门静脉分支灌注乙酸后,在门静脉、肝动脉内造成程度不等的栓塞,降低或阻断了相应肝组织的血供,其效果等同于门静脉、肝动脉双栓塞,为临床治疗肝癌提供了理论依据。
Objective To study the ultrasonic, pathological and biochemical changes of rabbits liver caused by acetic acid infusing into the branch of portal vein, and the pathogenesis of liver damage caused by acetic acid infusion. Methods Twenty - eight rabbits were randomly divided into five groups, 50% acetic acid was injected respectively into the branch of portal vein in all the groups. Results With different dose of acetic acid infused into branch of rabbits liver, endovasculitis, stenosis, thrombosis formed in the branch of portal vein and associated hepatic artery, loading to necrosis of liver tissue and deterioration of liver and kidney functions. The effects of acetic acid infusion into the branch of portal vein can result in embolization in both portal vein and hepatic artery, then decrease blood supply to liver tissue. The result is equal to the effects of embohzafion in portal vein and hepatic artery, it offers experimental evidence for treatment of liver carcinoma by selective embolizafion of portal vein.
