定量组织速度显像技术评价慢性肾功能不全血液透析患者左室不同步运动的研究

摘要 目的研究慢性肾功能不全血液透析患者的左室不同步运动。方法应用定量组织速度成像技术(QTVI)检测24例慢性肾功能不全血液透析患者(患者组)和26例正常对照组左室12个节段的室壁运动,并计算心脏收缩不同步指数,即收缩时间达峰值标准差(Ts-SD)、舒张早期达峰值时间最大差值(Te max-min)。结果以Ts-SD>33 ms为标准,患者组和正常对照组阳性率分别为87.5%和0;以Te max-min>100ms为标准,两组阳性率分别为62.5%和3.8%,两组间差异具有统计学意义(P<0.01)。结论慢性肾功能不全血液透析患者广泛存在左心室内收缩、舒张不同步运动。 Objective To study intraventricular dyssynchrony in maintenance hemodialysis patients with chronic renal failure. Methods Quantitative tissue velocity imaging(QTVI) was applied to study regional wall motion of left ventricle in 24 patients (maintenance hemodialysis patients ) and 26 normal subjects. Left ventricular systolic dyssynchrony index (Standard deviations of Ts, Ts- SD) and the difference ofTe (Te max - min) of all 12 segments were measured. Results With the standard ofTs - SD 〉 33ms, the masculine gender rate of patients group was 87.5% and the normal group was 0% ; With the standard of Te max - min 〉 100ms, the masculine gender rate of patients group was 62.5 % and the normal group was 3.8 % ; there was significant difference between the two groups (P 〈 0.01 ). Conclusion Systolic dyssynchrony and diastolic dyssychrony widely exist in maintenance hemodialysis patients with chronic renal failure.
