个体化综合治疗重症急性胰腺炎的研究

目的探讨重症急性胰腺炎(SAP)个体化综合治疗方法。方法将无胆道梗阻的SAP患者分为治疗组和对照组,2组内科常规治疗相同,治疗组在此基础上禁食不禁药,经胃管注入大黄、硝苯吡啶、谷氨酰胺,静滴复方丹参,外敷芒硝,腹水及肾衰者进行简易腹腔灌洗。结果治疗组全部治愈,对照组中转手术1例,死亡2例。治疗组禁食时间、住院时间显著短于对照组(P均<0.01)。结论根据病情采用个体化综合治疗,方法简单,疗效可靠,值得使用。

Objective It is to approach the method of patient individualized comprehensive treatment （PICT） to treat patients with severe acute pancreatitis （SAP）. Methods 23 SAP patients with no biliary tract obstruction were divided into two groups. 12 patients in the control group received only routine treatment, While 11 patients in the treatment group received both routine and PICT treatment. The PICT treatment includes proper doses of rhubarb decoction, Nifedipine and Glutamine injected into a patients＇ stomach through nasogastric tube, and herb Radix Salviae Miltiorrhizae injection formula through intravenous drip, and externally applied Mirabilite. Simple peritoneal lavage was used for patients with aseites or kidney failure, Results All patients in treatment group were cured, and their fasting time and hospital stay were significantly shorter （P〈0.01）. In control group, two patients died and one converted to surgery. Conclusion PICT is an effective and simple method to treat severe acute pancreatitis patients.
