乳腺巨大肿瘤21例治疗体会

Experience of treatment for 21 cases of giant breast tumor

目的探讨乳腺巨大肿瘤的临床特点,针对不同病理类型,选择不同术式,决定是否行一期乳房再造。方法收集中国医科大学附属第四医院普外科1995年3月～2005年10月收治的21例乳腺巨大肿瘤临床资料,根据原发肿瘤大小,术中快速病理检查结果及病人要求,选择肿物切除术、乳腺单纯切除术或乳腺癌改良根治术,手术同时行一期乳房再造术或行二期乳房再造术。结果肿物切除术11例、乳腺单纯切除术8例、乳腺癌改良根治术2例;一期乳房再造8例,二期乳房再造1例。结论乳腺巨大肿瘤术中确定为良性的,手术切除后行一期乳房再造是更好的治疗措施,减轻了年轻女性的精神负担,提高了患者的生活质量。

[Objective] To investigate the clinical characteristics of giant tumor of breast, according to their different pathologic types, determine modus operandi and whether making primary reconstruction. [Methods] From March 1995 to October 2005, 21 cases with giant tumor of breast were treated by our department. On the basis of tumor sizes, results of intra-operative quick freeze and requests of patients, choose tumorectomy, simple adenomammectomy, nodified radical mastectomy, and apply primary breast reconstruction or secondary breast reconstruction. [Results] Tumorectomy was applied to 11 cases, simple adenomammectomy was applied to 8 cases, nodified radical mastectomy was applied to 2 cases, primary breast reconstruction was applied to 8 cases, secondary breast reconstruction was applied to 1 case. [Conclusion] Primary breast reconstruction is a superior therapeutic method after giant tumor of breast has been excised and confirmed to be a benign one intra-operatively, it eases young woman of mental load, enhances their quality of life.
