VE导联对陈旧性下壁心肌梗死的诊断价值

Application of lead VE electrocardiography in diagnosis of the patients with old inferior myocardial infarction

目的观察VE导联的心电图特点，探讨该导联对陈旧性下壁心肌梗死的诊断价值。方法对100例正常成人和30例陈旧性下壁心肌梗死患者描计常规12导联心电图，并加做VE导联。比较两组人群VE导联的波形特点，分析该导联对陈旧性下壁心肌梗死的诊断价值。结果95．0％的正常成人，VE导联QRS波呈qR、R、Rs型，5．0％呈rS波，ST段位于等电位线，T波80、0％直立，13．0％平坦，7％倒置；93．3％的陈旧性下壁心肌梗死患者，VE导联QRS波呈QS或rS型，仅6、7％呈Rs波，ST段无偏移，T波70、0％直立，20．0％平坦，10．0％倒置。两组比较，以QRS波形态差异最为明显（p〈0．05或P〈0、01）。用VE导联QRS波呈OS型诊断陈旧性下壁心肌梗死敏感性为76．7％，特异性100％，阴性预测值93、5％，阳性预测值100％。结论两组心电图结果揭示了VE导联的心电图特征，VE导联呈QS型是诊断陈旧性下壁心肌梗死一项有价值的指标，

Objective To explore lead VE electrocardiographic characteristics and its value in diagnosis of the patients with old inferior myocardial infarction. Methods One hundred volunteers with no heart diseases and 30 patients with old inferior myocardial infarction had accepted routine 12 leads and lead VE electrocardiography examination, lead VE electrocardiographic characteristics in two different groups were compared. And its significance were analyzed in diagnosis of the patients with old inferior myocardial infarction. Results It was found that 95 % of the QRS complex was presented as qR, R and Rs, other 5 % showed rS, the ST segment was located at baseline ,and T wave was positive ,fiat and negative in 80% , 13% and 7% respectively in lead VE in volunteer with no heart diseases. It was seen that 93.3% of the QRS complex was presented as Qs or rS, other 6. 7% was Rs,the ST segment was no deflection,T wave was positive,fiat and negative in 70% ,20% and 10% in VE lead in patients with old inferior myocardial infarction. In QRS complex in lead VE, there was singificant difference between volunteer with no heart diseases and patients with old inferior myocardial infarction （ P 〈 0. 05 or P 〈0. 01 ）. Sensitivity,specificity,negative predictive value and positive predictive value of QRS complex to be QS in lead VE for diagnosis of patients with old inferior myocardial infarction were 76. 7%, 100% ,93.5% ,and 100% ,respectively. QS configuration in Conclusion Electrocardiographic characteristics in lead VE were showed. QRS complex to be VE lead is useful for diagnosis of patients with old inferior myocardial infarction
