≥80岁高龄老年人高血压与颈动脉顺应性的关系

摘要 目的研究≥80岁老年人高血压与颈动脉顺应性的关系,为治疗高血压和预防动脉硬化提供依据。方法参照ARIC动脉硬度指标测试方案,对≥80岁高血压患者49例(其中单纯性收缩期高血压患者25例,收缩期和舒张期均增高的高血压患者24例)进行高分辨率颈动脉超声检查,测量颈动脉收缩期内径、舒张期内径和硬度(包括紧张度、扩张性和僵硬度)并与对照组20例正常血压者进行3组之间比较及相关因素分析。结果颈动脉紧张度在对照组、收缩和舒张期高血压组、单纯收缩期高血压组3组之间两两比较差异有统计学意义(P<0.01)。颈动脉扩张性和颈动脉僵硬度在单纯收缩期高血压组与正常对照组、收缩和舒张期高血压组间比较差异有统计学意义(P<0.05),对照组与收缩和舒张期高血压组间比较差异无统计学意义(P>0.05)。结论≥80岁老年人单纯性收缩期高血压患者颈动脉僵硬度增加,颈动脉扩张性降低,临床上应重点关注对单纯性收缩期高血压患者的检查和治疗。 Objective To prevent arteriosclerosis and treat hypertension, we studied the relationship between carotid complaisance and hypertension in patients aged 80 and above. Methods According to the Atherosclerosis Risk guidance (ARIC) study, the carotid diameter and carotid stiffness, including arterial strain (AS), arterial distensibility (AD)and arterial stiffness(ASt) with high resolution nltrasonography were measured in 49 hypertension patient in 80 or above 80 years old, in which 25 case with only increased systolic We add 20 common people in 80 or above 80 years old and contrast difference significance among three group, Results Arterial strain among three group doesn' t have difference( P 〉 0.05 ), arterial distensibihty in isolated systolic hypertension (ISH) was lower than others, arterial stiffness in isolated systolic hypertension washigher than others( P 〈 0,05), Difference between systolic and diastolic phase hypertension group and common advanced age group were rot distinct. Conclusion The arterial stiffness was increase and distensibility was decrease, in isolated systolic hypertension patient in 80 or above 80 years old. we should pay attention to cure patients of isolated systolic hypertension.
