小瞳孔白内障人工晶状体手术方式的选择

Surgery of Cataract Extraction and IOL Implatation under Small Pupil

目的探讨小瞳孔下白内障摘除人工晶状体植入术的手术方式的选择和临床疗效。方法采用小切口非超声乳化白内障摘除人工晶状体植入术治疗小瞳孔并发性白内障32例（36眼），采用超声乳化白内障摘出人工晶状体植入术治疗小瞳孔并发性白内障20例（22眼）。结果术后矫正视力均较术前提高，0．5～0．8者27眼（46．55％），≥1．0者13眼（22．41％）；0．1～0.4者14眼（24．14％）。术后瞳孔形状均基本圆形，瞳孔直径较术前略大。结论小瞳孔下白内障人工晶体植入术手术复杂，手术方式的选择、病例筛选、手术时机以及并发症的处理等是手术治疗的关键因素。

Objective To observe the different surgical methods and effect of the complicated cataract under small pupil. Methods Total 58 eyes of 52 cases with small pupill and complicated cataract, small incision non phacoemulsification and IOL implantation was performed on 36 eyes of 32 cases, phacoemulsification and IOL implantation was performed on 22 eyes of 20 cases. Results The postoperative visual acuities were≥0. 5 in 27 eyes（46. 55% ）, ≥1.0 in 13 eyes（22. 41% ）. 0. 1 -0. 4 in 14 eyes（24. 14% ）. The pupil all return to round and more bigger. Conclusion Carefully observe, early dagnosis arid correct treatment can reduce the complications and improve the vision.
