Puncturing Biopsy of Cervical Spine Lesions Under CT Guide

探讨ＣＴ引导下颈椎病变穿刺活检的方法、途径、取材部位、穿刺针的选用及适应证。方法：对１０例颈椎病变患者，病变范围Ｃ１～Ｃ７，累及部位为椎体、椎板、横突、棘突、椎间孔，其中１例取坐位经口腔直视下取材，其他以侧卧或仰卧位在ＣＴ监视下取材。结果：１０例病人除１例骨巨细胞瘤放疗后活检未见肿瘤细胞外，９例均有明确病理学诊断，诊断正确率为９０％，无任何并发症发生。结论：ＣＴ引导下颈椎病变穿刺活检能清楚地显示颈部复杂的解剖结构及穿刺针位置，此方法操作简便，安全可靠，诊断正确率高，并发症极少。

Objective:To explore the puncturing biopsy method,path,sampling sites,puncturing needle selection and indications in diagnosing the cervical diseases.Methods:Ten patients suffering from cervical spine diseases were selected.The involved parts included vertebral bodies,archs,transverse processes,spinous processes and intervertebral foramena between C1～C7.One patient was sampled under direct eyesight via oral cavity in sitting position,and the others were sampled under CT guide in lateral or back decubitus.Results:Nine of 10 patients had confident histological diagnosis with the exception that 1 patient with giant cell tumor of bone was not found tumor cells in biopsy samples after radiotherapy.The accuracy of diagnosis was 90% and no comnplications appeared. Conclusion:The puncturing biopsy of cervical spine lesions under CT guide could clearly show the complicated cervical anatomical structure and determine the puncturing sites.It was proved to be a high accurate diagnostic method with rare complications.
