脂代谢紊乱在2型糖尿病肾病及其他慢性肾病患者中的临床研究 被引量:3

Clinical Study on Comparison of Dyslipidemia Between Diabetic Nephropathy and Chronic Kidney Disease Without Being Complicated by Diabetes Mellitus

摘要 目的探讨脂代谢紊乱与2型糖尿病肾病的关系。方法将41例2型糖尿病患者根据尿白蛋白排泄率分为正常白蛋白尿(N-UAlb)、微量白蛋白尿(M-UAlb)以及大量白蛋白尿(L-UAlb)3个亚组,将33例非糖尿病原因所致的其他慢性肾病患者根据24h尿蛋白定量分为轻度蛋白尿和中度蛋白尿两个亚组,分别测定空腹时的血甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)。结果L-UAlb组TC、LDL-C高于N-UAlb组和M-UAlb组,M-UAlb组TG、TC、LDL-C高于慢性肾病轻度蛋白尿组,L-UAlb组TC、LDL-C高于慢性肾病中度蛋白尿组,差异均有统计学意义(均P﹤0.05)。结论脂代谢紊乱与2型糖尿病肾病的发生、发展有关。 Objective To study the relationship between diabetic nephropathy (DN) and dys-lipidemia. Methods Fourty-one patients with type 2 diabetes mellitus (DM) were divided into 3 groups according to urinary albumin excretion rate (UAER): normalalbuminuria group; microalbu- minutia group; macroalbuminuria group. Thirty-three patients with chronic kidney disease(CKD) not being caused by diabetes mellitus were divided into 2 groups according to proteinuria:light proteinutia group and midrange proteinuria group. Triglyceride (TG),total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) were measured in the fasting state for all patients. Results TC,LDL-C were significantly higher in the patients with macroalbuminuria than patients with normalalbuminuria and microalbuminuria. TG,TC and LDL-C were significantly higher in the DN patients with microalbuminuria than CKD patients with light proteinuria. TC and LDL-C were significantly higher in the DN patients with macroalbuminuria than patients with midrange proteinuria. Conclusion Dyslipidemia is related to nephropathy in type 2 diabetic patients.
