椎管内神经鞘瘤误诊腰椎间盘突出症分析

Analysis on Intraspinal Neurinoma Misdiagnosed as Protrusion of Intervertebral Disc

以２例椎管内神经鞘瘤误诊为腰椎间盘突出症为例，对误诊的原因进行分析。认为忽略二者症状与体征的比较，忽略腰椎穿刺检查及脊髓造影是误诊的重要原因。脑脊液动力学改变和蛋白含量增高是椎管内肿瘤早期诊断的重要依据。椎管内碘苯酯或Ｏｍｎｉｐａｑｕｅ非离子化造影可以全面动态地观察椎管内神经鞘瘤，为临床鉴别提供有力证据。

In two cases of intraspinal neurinoma misdiagnosed as protrusion of intervertebral disc,the cause are analysed.The neglect of the comparison of symptoms and signs,and the lack of lumbar puncture and spinal myelography are the most important reasons of misdiagnosis. The changes of the dynamics of cerebrospinal fluid and the increase of the contents of albumin are the important basis for the early diagnosis of intraspinal tumor. With the intraspinal myelography of iophendylate or of Omnipaque,the intraspinal neurinoma can be fully observed,which can furnish the evidence for clinical diagnosis.
