圣乔治呼吸问卷对慢性阻塞性肺疾病患者生活质量差异的调查

Evaluating Life Quantity of COPD Patients with SGRQ

目的对COPD稳定期不同分级患者施行肺功能检测,应用圣乔治呼吸问卷(SGRQ)评测,以评估SGRQ评分在COPD病情程度分级间有无差异及其相关性.方法对符合慢性阻塞性肺疾病诊治指南诊断标准的169例COPD稳定期患者,进行肺功能测定,同时采用圣乔治呼吸问卷(SGRQ)自我评定.结果SGRQ3个部分分值和总分分值与FEV1占预计值%间均存在相关性,呈负相关.其中反映患者日常生活活动部分分值与FEV1占预计值%相关性最好(-0.716),总分分值与FEV1占预计值%的相关性也较好(-0.703),呼吸症状和疾病影响部分也有一定相关性.应用方差检验检测证实COPD0～Ⅰ、Ⅱ、Ⅲ级间病人的SGRQ3个部分分值(症状部分、活动部分、影响部分)和总分分值在分级之间存在差异(P<0.01).但Ⅲ组与IV组间SGRQ3个部分分值(症状部分、活动部分、影响部分)和总分分值在分级之间无统计学差异.其它各组组间两两对照均有显著性差异.SGRQ3个部分分值与总分分值的相关性均好,其中反映患者日常生活的活动部分分值和疾病影响部分与总分分值的相关性最好;活动部分和疾病影响部分间有相关性,而呼吸症状与活动受限和疾病影响间相关性最低.结论圣乔治呼吸问卷(SGRQ)评测COPD稳定期生活质量与肺功能及临床症状相关性良好,且能很好反映COPD稳定期不同病情的程度分级.

Objective To evaluate the association between the score of modified St George＇s Respiratory Questionnaire （SGRQ） and severity of COPD using lung function measures. Methods 169 outpatients or inpatients that matched the criteria of COPD according to Global Initiative for Chronic Obstructive Lung Disease （GOLD 2003） were included; each completed the modified SGRQ, and lung function measures. The subjects were divided into four severity groups according to FEV1 percent of predicted normal using the GOLD. Results FEV1 percent of predicted normal were related to total and the three sectional scores of SGRQ, the correlation coefficient were - 0.703 and - 0.716 in the activity section and total score. In ANOVA analysis, the differences of total and sectional scores between four groups were significant except the two groups of most severe disease. In the SGRQ score system, every partial score was related to the total score well, especially the Activity section and Impact section. The relationship between score of symptoms and score of imapct was less strong. ConclusionsThe results show that SGRQ score in COPD decreased with disease severity and deterioration of life quality. The data show a relationship between SGRQ and disease severity obtained by lung function. The SGRQ score can help in evaluating the severity of COPD.
