Treatment of Chronic Myeloid Leukemia with Unrelated Peripheral Blood Stem Cell Transplantation Mmismatched at a Single Locus

目的观察1个位点不合的非血缘关系外周血造血干细胞移植(URD-PBSCT)治疗慢性粒细胞性白血病的疗效及其并发症的防治.方法采用HLA配型1个位点不相合的非血缘关系外周血造血干细胞移植治疗慢性粒细胞性白血病1例.预处理方案为氟达拉滨+白舒菲+抗胸腺细胞球蛋白(ATG);输入非血缘关系供体单个核细胞数(MNCs)为5.60×108/kg,CD34+细胞为4.8×106/kg;采用霉酚酸酯(MMF)联合环孢素A(CsA)和甲氨蝶呤(MTX)方案,预防急性移植物抗宿主病(aGVHD).结果患者+25d造血重建,无aGVHD.结论MMF联合CsA和短程MTX可用于预防无关供体移植aGVHD预防的发生.

Objective To explore the effects and complications of treatment of chronic myeloid leukemia with unrelated peripheral blood stem cell transplantation mismatched at a single locus. Methods A patient suffering from chronic myeloid leukemia received transplant from an unrelated donor mismatched at a single locus. Conditioning regimen contained fludarabine, intravenous busulfan and antithymocyte globulin （ATG）. Transfusion values of mononuclear cells （MNCs） and CD34＋ cells were 5. 60 × 10^8 /kg, 4.8 × 10^6/kg. And Graft-versus-host disease （GVHD） prophylaxis consisted of mycophenolate mofetil （MMF） , cyclosporine A （CsA） and methotrexate. Result Successful engraftment was observed at day ＋25 and no acute GVHD was observed. Conclusion A combination of MMF, CsA and short-course methotrexate can offer an effective GVHD prophylaxis in transplant from unrelated donor.
