血清甲状激素变化与危重患儿预后的关系

The Relationship between the Changes of Plasma Thyroid Hormone and the Prognosis of Critical Disease

目的探讨PICU危重病患儿血清甲状腺激素变化与预后的关系.方法用放免法测定68例PICU患儿的甲状腺激素水平与疾病严重程度进行比较.结果非危重组患儿各项甲状腺激素水平与正常对照组比较差异无统计学意义(P>0.05),危重患儿评分越低,TT3、TT4水平下降越明显,组间差异有统计学意义(P<0.01);治疗后甲状腺激素水平明显高于治疗前(P<0.05),死亡组TT3和TT4含量明显低于存活组(P<0.01).结论危重病患儿病情越重,甲状腺激素水平下降幅度越大,TT3和TT4等的变化可作为评估PICU患儿预后的指标.

Objective To investigate the relationship between the changes of plasma thyroid hormone and the prognosis of critical patients in PICU. Methods Plasma thyroid hormone concentration were measured in 68 critical patientsin PICU by rediaimmunoassay, and they were compared with critical extent. Results There are significant difference in the level of plasma thyroid hormone between uncritical patients and normal control group, the lower grade of critical patients, the more decrease the level of TT3, TT4 （P 〈 0.01 ）. The level of thyroid hormone in patents after treatment was higher than that before treatment （P〈 0.05） , and contents of TT3 and TT4 in dead patients were obviously lower than that of survival patients （P 〈 0.01 ）. Conclusions The more critical state of illness of patients, the more decrease level of thyroid hormone. The changes of TT3 and TT4 might be a indication for evaluating the prognosis of critical patients.
